BOX 240773 ANCHORAGE, AK 99524 522-7773 677-7766 (FAX) October 3, 2016 Municipality of Anchorage Building Safety Division On Site Water and Wastewater Program 4700 Elmore Street Anchorage, AK 99507 Attention: Debbie Wockenfuss Subject: Ekvall Subdivision, Tracts A, B and C-1 S Case No. 12214 — Conditions to Plat Septic System Locations Dear Debbie: We have reviewed the conditions to plat for the development of Ekvall Subdivision into Tracts A, B and C-1 and have resolved the conditions by placing test holes on each of the tracts. A new septic system has been constructed to serve the house on Tract B. It replaced the existing system which was within the 150' separation distance from the Class C Well currently serving Tracts A and B. The existing system was decommissioned in accordance with Municipal Code. The inspection report for the septic system placed in 2008 under the supervision of Spurkland Engineering has been completed and is attached. The report was completed based on information obtained from the owner and the company currently providing maintenance on the system. The have stated that Bob Dean was the installer of the system. Records further indicate the system was inspected during construction by Lars Spurkland, the designer and also a professional engineer. Our field inspection along with data obtained from the sources mentioned herein resulted in the completion of the inspection report. No destructive inspection of the system was completed, but we are confident the configuration and size of the septic system is reasonably close. We cannot be entirely certain however since we were not onsite during construction of the system. Tract A houses a four bedroom single family home. It is served by a Biocycle Advanced Treatment System with a 25' long absorption trench. The maintenance reports are also attached. The alternate site for the septic system is shown on the Site Plan and a waiver from the State of Alaska Department of Environmental Conservation (ADEC) has been obtained. The home is served by the Class C well located on Tract A with a 150' protective radius. This radius encompasses nearly the entire surface area of Tract so a waiver was required for the alternate site. Test hole logs and percolation tests along with an as built of the subdivision is attached for your review. Ground water monitoring has been ongoing and was last measured on May 15, 2016. All test holes remain dry. A Well and Septic System Plan for the subdivision has been completed and is attached. The topographical information is also shown on the plan in the form of ground surface contours. A 16,000 square foot septic reserve area has been located on Tract C-1 which is outside the radius of the proposed private well for the lot. We have also shown the location and alternate site for the septic systems on Tracts A and B. The inspection report for Tract B is attached. Box 240773 Anchorage, Alaska 99524-0773 Re: South Anchorage-Ekvall Subdivision, Tract A-Advanced treatment using a Biocycle Type System- Waiver of Separation Distance to the Alternate Field from the Class C Public Water System-ADEC Plan Tracking Number 16 DWW 197B Dear Mr. Anderson, On June 23, 2016 a request for a waiver of a separation distance to a Class C public water system located at Ekvall Subdivision to allow the alternate field to be located 120 feet from the source well was received. You are recommending this waiver and feel that public health will not be impacted from the existing site conditions. A review of the resent samples and the well log and it's soils information along with the past waivers located on this development have been evaluated. The following waiver based upon the engineer's recommendation is herein listed. The separation distance to the future alternate disposal field on the same lot as the source well is established as 120 feet reduced from the 150 foot requirement. This waiver is based upon the engineer's recommendation and the engineering controls established upon the lot which assured the protection of public health. Should it be found that the site conditions endanger public health or are a public health concern,the waiver will be rescinded. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 410 Willoughby Avenue, Suite 303, P.O. Box 111800, Juneau,Alaska 99801, within 15 days of receiving the decision. A well is currently in place on Tract A which will continue to serve the house on Tract A and Tract B. Only one well will be required as a result of the subdivision. Tract C-1 could opt to tie in to the existing Class C Well for water service, but would require approval from the State of Alaska Department of Environmental Conservation. Field investigations indicate all of the wells in the immediate area were constructed without the presence of bedrock below the surface. All wells are cased to the bottom of the well. In all cases the wells produce a sufficient amount of water and range from 138' to 203' in depth. The majority of the wells are at an average depth of 200'. Static water levels in the existing wells vary from 75' to 143'. The production rates will be sufficient to support a five bedroom home and can be expected to be between 3 and 5 gallons per minute.
