The well that serves the primary residence had a sanitary seal and the well wires were in conduit.
The other well, which serves the small house near the road right-of-way, has a sanitary seal and
the well wires are in conduit completed by MW drilling on September 13, 2002.
We are requesting that your department issue final approval of the well and septic system serving
the primary residence so they can get their land use permit and enclose the structure prior to
snowfall.

To whom it may concern:
The septic system asbuilt drawings for the subject lot have been revised to reflect the following:
The well that was drilled in 1999 serves the old house that encroaches into the State DOT
road easement. The water line location is per Steve Redmond. The water line is
installed below the sewer service line to the septic system we are documenting. Per Steve
Redmond, the sewer line is all cast iron with shrink wrapped joints, was required by
ADEC. There are no inspections reports (by an engineer) or photographs to substantiate
this. The installer was Steve Redmonds brother, Bob Redmond.
· The well that was drilled in 1987 serves the log house, which is going to be renovated
(currently has a footing, slab, and no walls) once a land use permit is issued.

Dear Mr. Goodalh
Per your direction, thc property owner exposed the portion of the sewer service line where it
crosses the water service line. The water line was not professionally located, but the disturbed
soil in the area of the water service line excavation was easily discemable. All the pipe that was
exposed was cast iron and had shrink wrapped joints within 10 feet ofthe water line crossing. It
was apparent that the shrink-wrapping had been done during the initial sewer line construction.
It is reasonable to believe that the entire line is cast iron and shrink-wrapped, as claimed by the
property owner.
The well that serves the primary residence had a sanitary seal and the well wires were in conduit.
The other well, which serves the small house near the road right-of-way, was being worked on
(by a well driller) at the time of our inspection, and therefore did not have a sanitary seal on it.
The well driller is going to install a sanitary seal and insure the well wires prior to leaving the
site.
We are requesting that your department issue final approval of the well and septic system serving
the primary residence so they can get their land use permit and enclose the structure prior to
snowfall.

A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1987.
Permits are issued on a calendar year basis by authority of
Municipal Ordinance. A new permit must be obtained from this
Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the on-site
sewer system, the original as-built inspection report (three-part
form) must be sent to this office for review and approval, and
for documentation.'
Effective January l, 1988, a new fee schedule is in effect.
When re-applying for a new permit, the new fees are; $90.00 for
an on-site sewer permit; $50.00 for a well permit; $140.00 for
a combined sewer and well permit.

Dear Mr. Kennedy,
Attached is a certified copy of the log for test hole ~1. Water
monitoring for test hole to the north and to the west of this
test hole was done and no ground water was encountered. Due to
the well drained soil and the proximity of this test hole to the
holes previously monitored, I feel confident that no ground water
table exists in this area above the depth of 14'.
The existing septic system, as reported by Steve Redmond, consists
of a concrete septic tank draining into a drainfield. This system
has experienced no trouble throughout the period of its use.
The useable area for the placement of septic systems was indicated
to be 10 862 s.f. as stated in my letter to you dated May 19,1986.
This area should be reduced by 675 S.F. because of the driveway
that exists between the house and garage at the south boundary
of the property. Thus the useable area for septic disposal should
be adjusted to 10 187 s.f.
We request that a permit be issued to Mr. Redmond to construct
a septic system as shown. The system will not be connected to
any structures until a copy of the certificate to operate the
new well has been submitted to your office.

Dear John,
Enclosed please find a copy of the site plan for the above
referenced project. Also enclosed is a copy of the test
holes #1 and %2. The original test hole logs and the
original application for an on-site septic system permit
have previously been submitted to your office.
The concerns addressed by Susan Os%:a!~ and yourself during
our meeting at your office are as follows;
(1) Due to the set-back distances from the stream and the
existing wells in. the area, there is less than 4100 s.f.
of reserve area available for septic system replacement.
(2) The existing septic system on the southrwest
corner of the property is inside the well radius of the private
well. This is a non-conforming condition, and must be
addressed.
In order to adequa~y address these concerns, it is
proposed
that a new well be"'installed. This new well will be located
next to the log home in the center of the property, and will
serve both the existing house and the new log house. The
existing well in the south-west corner of the property will
be abandoned.
The abandonment of the existing well will provide an area
available for septic systems of 10 862 s.f. ( The total area
is 12 350 s.f. less i 488 s.f. for the buildings.) The
existing system would then be in compliance.
We hereby request a permit to construct the septic system.
A provision could be attached to the permit that no structure
will be connected to the system until such time as the existing
well is abandoned.
A permit for the new well will be obtained from D.E.C.
