It is mandatory, therefore the system be replaced to prevent a possible health hazard and damage to private property. We are therefore applying for a permit to construct a new septic system to serve the house. The Site Plan shows the location of the new system along with the location of the old absorption bed. It also shows the location of the well on the lot. The existing system was constructed in August of 1985 and included a separation distance waiver between the absorption bed and the lot line. We are proposing to replace the existing tank with a new 1,500 gallon STEP tank placed a minimum of 5' from the foundation. The absorption bed will be a minimum of 10' from the lot line. No waivers will be required. The testhole placed on the lot revealed silty gravel which percolated at a rate of 30 minutes per inch. Groundwater was encountered at a depth of 8', but was found at 6.5' during the monitoring period. We have therefore designed an intermittent bottomless sand filter with the piping placed 1' below original ground allowing for a 5.5' separation to the groundwater. The ground surface on the lot slopes from east to west at approximately 5% and is fairly flat from north to south. The bed will be constructed parallel to the slope in accordance with Municipal requirements. The new system will meet all requirements concerning separation distance from all wells in the area. Similarly, its placement will not affect septic systems currently in place. If the system is constructed in accordance with our design the following statements apply: The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. The system, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. SYSTI=-M CONSTRUCTION AND MATERIAL SPECIFICATIONS SUBJECT: LOT 213, RANSOM RIDGE SUBDIVISION GENERAL: 1. The scope of this project includes the removal of the existing 1,250 gallon septic tank and the procurement and placement of a new 1,500 gallon combination septic tank/lift station and pressure distribution system. Work also includes the construction of a new 36' long x 10' wide Bottomless Intermittent Sand Filter. A minimum of 2' of coarse sand material meeting the attached specification must be imported and placed in the filter area. The filter must then be constructed atop the imported material. Distribution piping must be placed at existing ground to provide 4' separation from groundwater. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The Contractor shall be responsible for obtaining all underground utility Locates and for the layout of the septic system. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. The property owner shall be responsible for revegetation of affected areas unless specifically agreed otherwise. Contractors installing wastewater disposal systems must be certified by the Municipal Department of Health and Human Services for system installations. Owners installing their own systems must receive prior approval from D.H.H.S. before beginning system installation. SFPTIC TANK/LIFT STATION INSTALLATION The 1,500 gallon septic tank/lift station is to be constructed by a certified tank manufacturer. Construction shall include an 18" manhole for access to the lift station. Lot 2B, Ransom Ridge Subdivision August 8, 1999 Page 2 2. The septic tank/lift station shall be sufficiently bedded to prevent settling or shifting of the tank. 3. All standpipes shall extend aminimum of 12 inches above final grade. 4. Septic tank/lift stations installed without 4' of cover shall have a minimum of 2" of direct burial insulation. A foundation cleanout shall be installed (}ne to four feet from the building foundation. Nocleanouts are required between the lift station and the drainfield in a pressure distribution system. 6. Final grading over the septic tank/lift station shall be such that a positive slope exists away from the septic tank. ABSORPTION BED CONSTRUCTION: 1. The absorption beds shown on the design. level. shall be constructed to the dimensions The bottom of the bed shall be within 2" of Distribution piping must be placed level with perforations down atop a level bed of drainfield rock. Rock should then be placed over the pipe to provide a minimum of 2" of cover. 3. A silt barrier or geotextile fabric must be placed between the drainfield rock and the natural soil backfill. Monitor tubes must be 4" in diameter and installed at the locations shown on the design. The portion in the drainfield rock must be perforated. Direct bury insulation must be placed over the distribution system when less than 3' of backfill depth is available. Finish grade over the bed must be mounded to prevent settlement or depressions. MATERIAL SPECIFICATIONS: 1. The lift station must be constructed by a Municipally approved septic tank manufacturer. An Orenco 20 OSI 05 HI-IF- 5 is recommended, Lot 2B, Ransom Ridge Subdivision August 8, 1999 Page 3 2. The following pipe materials are approved for use in septic system installations in the Municipality of Anchorage: Cast iron (perforated and solid), ASTM D3034 or PVC (Perforated and solid), ASTM F810 or HDPE (perforated, but not solid) and ASTM D2662 or ABS (perforated and solid). 3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Co. Styrofoam HI or equal). 4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco, or equal). 5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed between the final drain rock layer and the native soil layer. 6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing the #200 sieve. EQUIPMENT SPECIFICATIONS: Air Supply Line Shall be a Wasteflow Emitterline, 1/2" Inside Diameter from Anchorage Tank. Line shall be insulated with 1/2" foam pipe wrap inside a 2" Schedule 40 PVC Jacket buried a minimum of 3' below the surface. Air Compressor Shall be a Thomas Industries Model 5070 as supplied by Anchorage Tank. Compressor shall be located in the garage or crawl space. 3. Pressure Gauge from 0 to 10 PSI Shall be furnished and installed in the garage. INSPI-'CTIONS: A minimum of two inspections are required by Municipal Ordinance. These inspections must be conducted under the supervision of a professional engineer registered in the State of Alaska. The first inspection must be conducted after the excavation of trenches, beds or pits and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled. No backfill should be in place at the time of the second inspection. The contractor is responsible during the layout of the system to assure that all separation distances from adjacent wells in the area are met. The reason for bringing this to your attention at this time is due to concern 'from a lending institution regarding the setback lines which, really don't apply as shown on the ~iled plat. The creek has been diw~rted and placmd in a pipe in a new alignment north o¥ the location shown on tbs plat. We feel the 100' septic setback lines should relate to the current alignment of the cresk/pipe and not the incorrect lines as now shown on the filed plat. ~]£~L~D~!i At the time of tbs -Filing of the plat for Lots 2A and 28 of Ransom Ridge, a note was added to the plat which rsads as follows: "No septic system will be allowed in the 100 ¥0ot setback creek septic protection area." In addition to the abov~ n~e, the setback lines bassd upon the location of the creek at the time of filing were shown on the plat (see attachment #1, a copy of '/he filed plat). Submequsnt to the plat filing, the ADOT/PF has reconstructed Golden View Drive in the area of 'the Lot 2B. The work involved 1) Golden Visw Drive vertical alignment changes which raised the centerline grade which caussd 2) a diversion of a large majority of ~he previous creek water which previously passed through this property to a new area along the east side of Golden View Drive, which resulted in, 3) the installation of an underground pipe from the west slops of Golden View Drive to Ransom Drive to handle any seepage water through the roadway fill area. In consideration of the mutual covenants contained herein,the parties to this Maintenance and Repair Agreement agree as follows: 1. Advanced Wastewater Treatment Systems. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as ISS � located at(legal description). OWSows R. Any change to the design or function of an AWWTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Certificate of On-Site Systems Approval (COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code (hereinafter,"AMC") 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On-Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. 1 eriii. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations, Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS,the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement. and any renewals thereof, at the owner's sole expense, to pay for any and all: (I) repair(s),(2) maintenance, (3)adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality,the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor,the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On-Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties,with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this agreement,the only authorized representatives of the parties are: a. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 8. Jurisdiction: Choice of Law. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Maintenance and Repair Agreement. 9. Severability. Any provisions of this Maintenance and Repair Agreement decreed invalid by a court of competent jurisdiction shall not invalidate the remaining provisions of the Maintenance and Repair Agreement.
