Wastewater Disposal Regulations ( 18AAC72) and Drinking Water Regulations ( 18AAC80 ).
3. The engineer must notify DHHS at least 2 hours prior to each inspection. Provide notification by calling
(907) 343-4744 (24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
The system may be redesigned to utilize the lower GP layer with verification of soils. DHHS shall be notified of any
design change.

My finn was contacted to design and install a replacement septic system for the referenced lot. The existing drain field
(a cesspool) is in failure. 1 conducted a field investigation to locate existing wells and septic system as well as a record
search before designing the proposed system. We conducted a soils investigation in the most likely location for a
replacement system. The cesspool is located 97 feet from the existing well. The most likely place for a replacement
system is located approximately 80 feet from the existing well. This design proposes drilling a new well on the
northern side of the house and abandoning the existing well in accordance with ADEC Regulations before installing
the new soil absorption system. The new system will be located approximately 35 feet from a 12 foot tall, 2 Horizontal
to 1 vertical slope to the south. The soils are silty sands and gravel and effluent leaving the system will not daylight.
The lot is approximately 29,250 square feet in size. Lot 10A slopes to the south at a rate of approximately 3 to 15%
percent. The proposed holding tank installation will be located in the southern portion of the lot on a relatively flat
area south of the existing house. There will be easy access to the tank for pumping trucks. The proposed location is
greater than 100 feet away from any existing or proposed wells. The proposed system will be greater than 10 feet from
the water service lines. The proposed installation will not affect the future development of the surrounding or existing
lots. See the attached design. The owner is an experience contractor and plans to install the soil absorption system
himself. He will hire a certified well driller to install the well.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Engineer's Comments: This Investigation was completed in compliance with ADEC and MOA regulations. The
assessment of the condition of the well and septic applies only to the conditions as of the day tested. The flow
and absorption rates may change due to subsurface conditions that may not be observed from the surface,
changes Inland use, local soil characteristics, groundwater levels that may fluctuate during the year and the
water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the
evaluator of the well and septic system. Therefore,
ArcTerra can not give any estimate of how long a
system will function satisfactory for current or future
occupants or can ArcTerra guarantee that no unseen
encroachments, deficiencies or discrepancies exist.

RE: Encroachment: Septic Absorption Trench BLM Reserve
US Survey 3200, Lot 10A, Grid SW4455, 165 Chickweed Lane
The Right of Way Division has reviewed a request for a letter of non -objection to an existing septic
absorption trench encroaching 5' into the 20' BLM Reserve. On the as -built survey dated March 1, 2010
(Robert E. Johns, Jr. & Assoc.), submitted with the request, the petitioner has shown the encroachment.
This letter of non -objection is issued with stipulations, and by using it the petitioner is agreeing to the
following:
1. Municipality of Anchorage (MOA) will be held harmless, now and forever, for any damages or
injury to any person as a result of the encroachments.
2. All applicable codes and regulations will be observed and maintained within the easement.
3. This letter of non -objection will in no way preclude MOA from full use and enjoyment of its
rights within any portion of the easement.
4. Additional and extraordinary costs incurred during any future required construction, repair or
reconstruction of MOA improvements to accommodate any or all of the encroachments shall be
paid by the property owner.
5. Maintain letters of nonobjection from the following utilities:

® Chugach Electric Association, Inc.

❑ Municipal Light & Power
❑ Eyeeom (Girdwood Cable TV)
® Alaska Communications Systems
