Emergency Tank Replacement Permit
Gentlemen:
My tirol was coutacted to investigate a collapsed tank at the above refereuced lot. Sewage is backing up into tile house,
and is being pmnped on a daily basis. The attached site plan shows tile proposed location of the replacement tank.
Tile lot is approximately 1.25 acres in size. [,ol 3A slopes to tide smttbwest at a rate of approxilnately 1-3 percent. The
north eastern poltion of the lot rises al)proximately 5 feet and fl~en levels off. The proposed installation will be located
on the aortbem portion of the lot. The existing 1,250 galloo septic tank will be crusbed ill place. The cxisting fiekl
will be reused. Double clean-outs will be installed after the tank. Tbe proposed location is greater than 100 feet away
from the existiug well serving tiffs property and 25 feet fi'om the water service liues. The surrounding wells m'e located
greater than 100 feet from the proposed installation. The proposed inslallation will not affect the fltture development
of the SmTouudiug or existing lots. See tile atlacbed design.

Ladies and Gentlemen:
Your departmeat issued Permit SW990082 for tile replacement of the septic tank at tbe above referenced property.
Daring the installation oftbe tank, tile owner requested my firm to investigate upgrading his drain field. It is cun'ently
operating h~ a surchalged rammer. We conducted a single test bole on May 6, 1999. We found tile soils to be silts to
eighteen feet. We did not encoonter m~y ground water or bedrock. The attached soils log and percolation test attached.
Tile lot is approxi~nately 1.25 acres ill size. Lot 3A slopes to tbe soutbv,,est at a rate of approximately I-3 percent. The
nollb eastern portion oftbe lot rises approximately 5 feet and then levels off. The proposed installation will be located
on tbe nm-them portion of the lot. The existing 1,250 gallon septic tank was replaced under the original permit on
5/5/99. Tlm ex/stir, g field wi!! be l'euse,'!. Double c!em~-outs will be installed after the tattle,. The proposed location is

A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1988.
Permits are issued on a calendar year basis by authority of
M~nicipal Ordinance. A new permit must be obtained from
this Department for any well and/or on'site sewer system
not installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the
on-site sewer system, the original as-built inspection report
(three-part form) must be sent to this office for review and
approval, and for documentation.
When applying for a new permit; the· fees are: $90.00 for
an on-site sewer permit; $50.00 for a well permit; $140.00
for a combined sewer and well permit.
