Mclntyre Tract B-1 Septic System and Well Permit Application Dear Mr. Crewdson: We respectfully request your review and approval to construct the subject well and septic system. Attached to the application sheet, including this letter and sizing and specifications requirements are the following documents. 1. Well Locations and Septic System Layout stamped and signed by a licensed professional engineer. 2. Design and calculations stamped and signed by a licensed professional engineer, shown in Figure 6. 3. Results of soils, percolation, and water table test stamped and signed by a licensed professional engineer. This septic system design is for a five-bedroom home to be served by a private well. We propose installation of a 1500-gallon septic tank, a bed absorption system and new well. The attached Well Locations and Septic System Layout are provided in Figure 1 and shows existing adjacent and surrounding property well and septic system approximate locations and the proposed well for this lot as well as the location of soil and percolation test pit used for this design application. No other wells and surface water exist within 100 feet of our proposed septic system design. The included percolation and soils identification logs reflecting exploration to a depth of 16.0 feet and 16.5 feet was performed and stamped by a registered professional engineer in the State of Alaska. The test pit locations are shown in the attached figures. The test indicates well graded gravel. The soil percolation rate was faster than one minute per inch. No water or bedrock was encountered and no water was recorded in a four-month span of monitorh~g. Developed surrom~ding lots are served by on-site septic systems and private wells. Soil percolation rate faster than one minute per inch requires a two-foot thickness of filter material underlying sewer rock gravel and 150 square feet of septic area per bedroom. The septic system design area requirement for five bedrooms is 750 square feet; at 15 feet wide, the required bed length is 72 lineal feet. The components and materials and the installation shall meet MOA's Handout for Typical Bed Requirements and Specifications per AMC 15.65, attached. The topography is generally sloping variably from north to south at approximately 15%. Surfac~e~ waters are more than 100 feet from the proposed and alternate septic field locations. This proposed installation and well location create no adverse effects or ,limitations on existing or future adjacent wells or septic systems under current MOA Code. Indemnification. Grantee hereby defends, indemnifies and holds Grantor harmless from and against all claims, demands, losses, liabilities, damages or expenses arising out of or in any way related to the exemise of the rights under this Agreement. 3. Use of Easement Area by Grantor and Grantee. Grantee shall be solely responsible for the maintenance and upkeep of the water well maintenance ingress and egress easement area. Grantor shall not use the area in any manner inconsistent with the rights granted to grantee hereunder. 4. Running with the Land/Binding Effect. The agreements contained herein and the easement and other rights granted hereby shall run with the titles to the Grantor Property and shall bind and inure to the benefit of the parties hereto and their respective heirs, successors and assigns.
