SEWEeKK RCiWA1TER All work under this project shall comply with current Municipality of Anchorage (MOA) Regulations. The scope of this project includes the installation of a new 1300 gallon HDPE septic tank, a "REQ sTwEs 550 gallon Fat Albert S.T.E.P. tank, and a new I.D.S.F. septic system to serve the proposed four bedroom residence located on the referenced property. WfUMPEOT" SITE INVESTIGATION aFWMTEST The Contractor must carefully examine the project documents, have full knowledge thereof, have investigated the site, and be satisfied with the conditions affecting the work. General The work under this section consists of the performance of all operations pertaining to famishing and installing the septic tank/lift station, air compressor and piping to the sand filter. B. Materials 1. Tank a. Septic tank shall be a two compartment tank of at least the minimum size indicated on the drawings and MOA approved. b. First compartment shall have a separate 4" pump -out riser. The second compartment shall have a separate 4" pump -out riser. 2. Manhole Riser or Lift Station Tank a. An additional second compartment manhole riser shall be capable of being equipped with the following: 1. A junction (Nems 4x) box or equal, bonded or attached to the pump basin. 2. UL listed electrical cord grips, installed in the J -box. -OR- b. An additional tank (M.O.A. approved) may be provided for the effluent pumping assembly and the items in (a.) above. 3. Effluent Pumping Assembly, Controls, Alarm & Piping a. Effluent pumping assembly shall be of approved materials, including the following: 1. A 110 volt or equal U.L. Listed effluent pump. 2. High-density PVC cylinder housing the pump, with level controls and screen, serving as a baffle to prevent the screen from clogging. Pape 7 Lot 5C;111amna Acres Subdivision May 30, 2006 3. All piping shall be PVC or other non -corroding material. b. Pump controls and alarm system shall be an approved system with the following options consisting of: 1. Event counter 2. Elapsed time meter 3. Program timer 4. 10 wattheater 5. Remote alarm panel option with a minimum of 80 DB sound pressure at 24", operating temperature 30AC to 65AC, continuous sound to be located in the home. 6. Oil -tight visual alarm with push -to -silence feature. Automatic audio -alarm reset. 7. 15 amp motor rated toggle switch, double pole, double throw with three positions; manual (man), automatic (auto) and center (oil) (H.O.A.). 8. Nema 4x -rated, fiberglass, or equal, enclosure with hinged cover. 9. All controls to be installed as per manufacturer's recommendations. 10. Control panel to be installed on outside wall of home within sight of septic tank/lift station. c. Piping from the septic tank to the sand filter is to be 1 1/4" PVC schedule 40 solvent weld piping properly bedded as described in Materials Specifications and sloped to drain back to the lift station. Insulation board 2" thick and T wide centered over pipe is to be placed where soil cover is less than 4' depth over pipe. d. The airline from the house to the filter is to be HDPE arctic insulated pipe 3/4"-2" SDR - 11. The compressor will have a gauge (0-10 psi) and be extended into the living area or garage for easy monitoring by home occupants. Pape 4 Lot BC; Illamna Acres subdivision May 30, 2008 4. Air Compressor a. Air compressor shall be a THOMAS LINEAR SERIES model number 5060-A or 5070V -A or M.O.A. approved equal. CONSTRUCTION SEQUENCE In order to insure proper installation, the system is recommended to be installed in the following order: I. Sewer line from house to tank 11. Septic tank/lift station III. Sand filter IV. Leachfield bed (if required) V. Final grading CONSTRUCTION 1. Construction Instruments Contractor shall provide instruments such as transits, or levels, for establishing proper elevations and levelness of pipe or materials. 2. Pipe Laying All pipe shall be laid with material free of large or sharp rocks and organic matter. Each section of pipe shall be handled carefully and placed accurately. The spigot end shall be fully inserted pointing in the direction of flow and glued in place to make a water tight seal. Each section of pipe shall be properly supported to insure true alignment and an invert which is smooth and free from roughness or irregularity. At all times, when work is not in progress, open ends of pipe and fittings shall be closed so that no undesirable substance will enter the end of the pipe or fittings. Papa 5 Lot 5C; 111amna Acres Subdivision May 30, 2006 3. Bedding of Pipe for Sewer Lines The pipe should be bedded with materials free of large or sharp rocks and organic matter, compacted. Native materials may be used as back fill material above the pipe, provided it is free of large rocks and organic matter. All lift stations shall be installed in accordance with the manufacturer's recommendations and standards. Electrical installation is to be in accordance with the National Electric Code (latest adopted edition) by a licensed electrical contractor, with M.O.A. inspection where required. If MOA inspection is not required, a licensed electrician is to provide a letter to the Engineer stating that the system meets or exceeds all applicable codes. 4. Excavation Excavate to proper depth to allow gravity flow, install tank level, tolerance 0.05 foot, on undisturbed or compacted classified sand fill material. Create a level sand surface over the tank and add 2" burial foam +35 PSI directly over tank. Mound unclassified soil over tank with a minimum of 2 feet of cover and anti -flotation weights that may be required in areas of shallow ground water. 5. Connections All connections to be as per MOA requirements, PVC 3034 piping with Caulder or equivalent couplings for inlet riser pipes. All tank connections are to be completely water tight. Materials 1. Sand Filter a. Work Description: Single-family home construction with associated driveway, well, septic and minimal yard clearing. All work will be performed in accordance with the attached plan, sheets 1-2 of 2, dated October 5, 2006. Project Location: Lot 5C, Iliamna Acres Subdivision, 5345 Riverton Ave, Anchorage, AK. Anchorage General Permit Conditions: General conditions: 1. Water Quality Certification. You must comply with all conditions specified as part of the Alaska Department of Environmental Conservation Water Quality Certification, which is part of these General Permits (GPs). 2. Historic and Archaeological Sites. These GPs do not authorize activities, including the use of material borrow sites, in or that adversely affect known historic properties listed or potentially eligible for the National Register of Historic Places, nor any such sites listed or found to be potentially eligible in the future. The pernitee shall stop work when cultural resources are Inadvertently discovered during the course of work and notify the Corps. The Corps will enter into consultation with the Alaska State Historic Preservation Office to determine if historic properties will be affected by the permitted work. Work shall resume after cultural resources have been evaluated pursuant to Section 106 of the National Historic Preservation Act. 3. Inspections. You must permit the District Engineer, or his designated representatives, to inspect the authorized activity at any time deemed necessary to ensure that it is being, or has been accomplished In accordance with the terms and conditions of these GPs. 4. Maintenance. You must maintain the authorized activity in good state, and in conformance with the terms and conditions of these GPs. You are not relieved of this requirement even if you abandon the authorized activity, although you may make a good faith or legal transfer to a third party, as described below. Should you wish to cease to maintain the activity or abandon it without a good faith transfer, you must obtain a modification of the GP authorization from the Corps Regulatory Branch either directly, or through Municipal Planning Staff. Such modification may require restoration of the area. 5. Sale or Transfer of Property. If you sell or transfer the property associated with the authorized activity, you must obtain the signature of the new property owner as the transferee in the space provided on this document and submit a copy of that page to the Municipal Planning Department and/or Corps Regulatory Branch, along with appropriate documentation of the transaction. You must submit this information within 30 days of the property transfer, to validate transfer of the GP authorization to the new party. Where there are wetlands along or around a stream or waterbody, the project shall Include and maintain a non -disturbance setback, measured horizontally from the ordinary high water mark, as specified in Table 4. For previously unplatted residential subdivisions, the setback area shall be traded out, rather than being included in Individual lots. Notes: a. Applicants should contact Municipal planning or Corps Regulatory staff to obtain or confirm delineation of the setback: b. For currently unmapped waters, standard setbacks shall be: L 100' from streams supporting anadromous fish species, whether catalogued by the Alaska Department of Fish and Game, or otherwise so documented; ff. 85' from headwater streams, defined as those reaches upstream of the point at which the average annual flow is less than 5 cubic feet per second; hi. 85' from springs, defined as areas of permanent or seasonal discharge of groundwater that serve as a source for streams ordrainageways; iv. 65' from lakes, ponds or similar watorbodies having a minimum permanent surface area, at ordinary high water, of 2,500 square feet; v. 65' from streams that are neither anadromous norheadwaters, as defined above; and, vi. 25' from natural or constructed drafnageways, as defined and/or mapped by the Municipal Watershed Management Section. 2. Construction Timing Window. To protect nesting migratory birds, as required by the Migratory Bird Treaty Act, no work (i.e., clearing, grading, excavation, stockpiling, or placing of fill) may occur on the site between April 15 and July 15, unless: a) the site is sufficiently disturbed prior to April 15 to eliminate suitable nesting habitat; and, b) construction activity will occur continuously through June 15. Note: Clearing or other disturbance of the site prior to April 15 may require Corps or other authorization. 3. Surface Water Features. The project shall Incorporate design features and construction methods to maintain flow of surface water across the site. To accommodate this requirement, the applicant shall assess the project site to Identify drainage patterns and previously unmapped surface conveyances, such as streams and drainageways. These features must be field verified by Corps Regulatory, or Municipal Planning or Watershed Management staff. The applicant shall contact the Office of Management and Permitting (tel.269-8690), Alaska Department of Natural Resources, if the proposed project will cross any surface flowing waterway. Surface flow features shall be clearly depicted, along with any appropriate setback(s), on the proposed plans submitted with the GP application, as shall the measures proposed to maintain such flows. Cross -drainage may be directed around the development area, but the project must maintain downstream flows similar in quality, quantity and direction to the pre -project condition. As discussed under Setbacks, the GPs may allow crossings of unmapped waterways, but do not authorize permanent diversions, channelization, or other alteration. 4. Local Flooding. The project shall incorporate design and construction features/methods necessary to ensure that the proposed project will not cause flooding of adjacent properties or public Infrastructure. 5. Stonnwater Functions. The applicant shall demonstrate that the project plans Include sufficient measures to effectively replace the site's pre -project storrnwater retention and filtration capacity. The measures shall be sufficient to approximate zero net runoff Increase for storms with 2 -year frequency and no more than two times net runoff increase for storms with a 10 -year frequency, Including flows from subdrains. Verification that the project's stonnwater design meets this criterion must be obtained from the Municipal Welland Management Section or Its designee, and must be Included with the GP application package. If you need more time to complete the authorized activity, submit a request for extension under these GPs to the Municipal Planning Department prior to the expiration date. Any change to the design or function of an AWWTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code(hereinafter, "AMC") 15.65.These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit.An On-Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA,and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations,Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS,the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement,and any renewals thereof, at the owner's sole expense, to pay for any and all: (1)repair(s),(2) maintenance, (3)adjustment(s), (4)replacement costs,and(5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws. regulations, rules and orders for the AWWTS. C. Upon request by the Municipality,the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor,the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On-Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties,with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this agreement,the only authorized representatives of the parties are: a. Owner: LEANN DAMRON b.
