Approval of the plat for 18 months subject to the following conditions:
1. Resolving utility easements.

Installation Requirements for Septic Tanks in the.
Municipality of Anchorage:
Steel - Steel septic tanks shall be placed on undisturbed soil at the
bottom of the excavation for the tank and backfilled with a granulcr
material free of Jorge rocks or boulders to a maximum depth as_
prescribed on the exterior of the septic tank. The tank may then be
backfilled with 12 -inch maximum layers of -granular soil and trampled
evenly following the placement of each layer.
• H.D.PZ Premier Plastic tanks shall be placed on compacted or
undisturbed bedding of sand/gravel mixture or clean granular soil:
Allow clearance around the tank to properly place and compact backfill'
around the lower half of the tank. --All Premier Plastic tanks shall have
e vertical length of 4 inch perforated drainpipe beside the tank
extending to the bottom of the tank and up to grade to monitor the
ground water table. The tank may then be backfilled with 12 -inch
maximum layers of granular soil and trampled evenly.
Fiberglass Tank - Excavate at least twenty-four inches beyond all
sides of the tank to allow room for proper compaction. Set tank level"
on a minimum 4 inch thick compacted sand or granular bed overlying a
firm compacted and uniform base. Backfilling is a layering process
that involves backfilling, compacting the backfill, and filling the tank
with water to a level just above the compacted backfill layer. Backfill
should have a uniform gradation and be free of any deleterious
materials such as stones that are more than 2 1' in diameter.
Typically to crushed rock or pea gravel is preferred. Each backfill
layer should be uniform, no greater than 24 inches thick, and of nearly
equal height around entire perimeter of the tank

3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade.
4. Septic tanks installed with less than 4 R. of cover shall be insulatgd.
S. A foundation cleanout shall be installed one to four feet from the building foundation. In
the line between the tank and the lwchfield there shall be two adjacent eleanouts (unless
an effluent pumping system exists within the septic tank). These eleanouts shall be
located on undisturbed soil not more than 10 R. from the tank. The first cleanout, in line.
shall be to clean toward the leachfield. The second cleanout -shall be to clean toward the
septiotank.
6. Final grading over the septic tank shall be such that a positjye slope exists away from the
septic tank.

ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
1. Excavate the proposed treaeh to the dimensions shown on the design. The bottom of the,.
excavation shall be within 2 inches of level. If the sidewalls of the excavation become
smeared, .they must be raked or scratched (ruffed -up) before gravel (sewer rock)
placement.
2. Once the :gravel is installed, the distribution pipe is to be installed level with the
perforations faced downward. Gravel is then to be placed over the distribution pipe to
provide a minimum of 2 incherofcover over the pipe.
3. A silt barrier must be installed between the final gravel layer and the native soil backfill.
Ensure the silt barrier covers the entire gravel surface before placing backfill.
4. Monitor tubes shall be of four (4) inch diameter, installed approximately in the locations
shown on the design, and extend a minimum of 12 inches above final grade. The portion
of the monitoring tube extending through the gravel shall be perforated from the bottom of
the trench to the invert of the distribution pipe. This is equivalent to the effective depth of..
the gravel as noted on the design.
5. Backfill over the final gravel layer must not be less 11= twenty-four (24) inches.
Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The.
finish grade over the trench must be mounded to prevent the formation of a depression
after sealing. .

1. Any septic tank proposed for installation must be constructed by a Municipal approved
septic tank manufacturer.

3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemipal
Company Styrofoam M or equal).
4. Septic tank inlets and outlets shall be fiaed with watertight couplings (Caulder, Femco, or
equal).
5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed
between the final leachfield gravel layer and the native soil backfill.
6. All leachficld gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3%
passing the 0200 sieve.
7. When sand is being used as a filter material, its gradation specifications must conform.to
current M.O.A. or D.E.C. requirements, which ever requirement applies.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the installation of the
wastewater disposal system. These inspections will occur as follows: .

Dear Mr. Sweeneys
This Is in response to your November 27, 2002 application for a Department
of the Army (DA) permit to construct a culverted crossing over Little
Survival Creek. The project is located at the end of Betty Street in the NW
W, SE U, Section 3, T. 11 N., R. 3 W., Seward Meridian, Latitude 61. 04. 04-
N., Longitude 149. 47' 00' W. in Anchorage, Alaska.
Department of the Army permit authorization is necessary because your
project would involve placement of dredged and/or fill -material into waters
of the U.S. under our regulatory jurisdiction.
Based upon the information and plans you provided, we are hereby verifying
that the work described above which would be performed in accordance with the
enclosed plan (sheets E1-5)), dated 24 February 2003, may be done under the
authority of nationwide permit (NWP) #14, Linear Transportation Projects,
which is described in Enclosure 1. you must comply with all terms and
conditions listed on Enclosure 1, as well as to the special conditions listed
below:
1. The culvert will be installed following diversion of the stream around the
work site to prevent sedimentation of downstream areas.
2. A 48^ z 24' culvert will be installed in the manner Alaska Department Fish
and dame required.
3.The Anchorage Debit/Credit Method shall be used to determine the amount of
mitigation required. A conservation easement along Little survival Creek
will offset the debits.
Further, please mote General Condition 14 requires that you submit a
signed certification to us once any work and required mitigation are
completed. Enclosure 2 is the form for you to complete and return to us.
This verification will be valid for two years from the date of this
letter, unless the NWP authorization is modified, suspended, or revoked.

2. Resolving with On -Site Water and Waste Water information to satisfy the
requirements specified in AMC 21.15 and AMC 15.65. The information
must be submitted for each lot of this proposed subdivision. This
information must include, but may not necessarily be limited to:
• Soils testing, percolation testing and ground water monitoring
must be conducted to confirm the suitability for development using
on-site wastewater disposal systems. Ground water monitoring
must be conducted during high ground water season in either the
fall (October) or spring (April -May).
• Areas designated for the original and replacement wastewater
disposal system sites must be identified and must meet all criteria
specified in AMC 15.65, including slope and setback requirements.
• Topographical slope information must be submitted.
• Supporting documentation on water availability must be provided.
3. Eliminating the 30 -foot road easement on the western property boundary if
being provided via this plat. If the easement exists of record, or vacate the
easement if the Traffic Department concurs.
4. Resolving with the Traffic Department, the need to continue the road right-
of-way to the east property line, for future connection to Betty Jean Street.
5. Providing a 25 -Coot Stream Maintenance and protection easement along
both sides of the stream as required by AMC 21.80.040.
6. Showing the wetland boundary on the plat and the class designation.
7. Placing the following notes on the plat:
a. Portions of this property are affected by Class B wetlands. A
permit from the Army Corps of Engineers is required for work
within the Class B wetlands portion of this property.
b. No structures; fill; paving; vehicles, or other materials may be
placed within the 25 -Coot stream maintenance and protection
easement. No vegetation may be cleared or otherwise significantly

disturbed within the 25 -foot stream maintenance easement.
Prior to the issuance of a building or land use permit, the applicant
shall provide a site drainage plan for the area affected by the
application, including an appropriate drainage outfall for surface
water and roof drainage. The drainage plan shall also indicate
effects if any, on adjacent properties.
8. Resolving with Parks and Recreation, the location, size and alignment of
the requested public use easement along the outer 10 -feet of the stream
maintenance and protection setback.
9. Resolving with Army Corp of Engineers, the need for a new Corp permit
for the culvert crossing which is not in the same location as the one
previously permitted by the Corp, to The Dean Company, or show the
proper location of the culvert crossing already approved by the Army Corp
of Engineers. A sign -off from the Corp shall be on a copy of the final plat
to ensure that the location is appropriate.

Proposed Woodhaven Subdivision
The purpose of the attached Preliminary Plat Application is to create two lots from the above referenced
property. This remainder tract has previously been submitted for subdivision into four lots and been deniedby
the Platting Board. The primary issue behind the denW was access to the proposed lots. Also, the surrounding
community had objection with the extension of Betty Street and the effect a new road would have on the
adjacent lots to the west.
The petitioner has redesigned the proposed subdivision to create only two lots from the remainder tract The
Petitionerhas used the comments from the Community, MOA Planning, MOA Traffic, and The Fire Department
to create this design and resolve the access issue. The design of the Flag lotwill provide access and frontage to
both lots as required by municipal code. The Public Use Easement and constructed drive will provide physical
access to Lot 2 and supply adequate tum -around space for emergency vehicles. This design solves the access
issue and has no impact on the vegetative buffer between the existing lots to the west.
Forthe reasons mentioned above, as the property owners representative, we respectfully request approval of
Woodhaven Subdivision.
