Jerry, this confirms our conversation and requests assistance from Planning regarding 2 plat notes Inconsistent with the
referenced project
PME had designed a renovation/addition to the existing facility In 1999/2000. Access for the addition was to have been off
Rabbit Creek Road. Preliminary ADOT approval for a driveway permit was received on 2/21/00. Final ADOT approval was
contingent on receipt of the performance fee which was not paid. The construction project was later put on hold at AFD's
request Subsequent to this the parcel was replatted. Plat notes 3 and 5 relate to access onto Rabbit Creek Road. AFD
recently made the decision to proceed with the project
PME requests assistance from Planning In obtaining the necessary approvals to allow us to construct the FS 10 project
with access onto Rabbit Creek Road. The charge code for your services will be 431 -1612 -ACCT -161205-1999. -
