Septic System Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install an upgrade septic system be issued for this property. The
proposed systems will serve an existing four (4) bedroom house. Currently the lot is developed. The
existing system is in failure and will be re -used as a reserve. The proposed system will utilize a 1250g
septic tank and conventional drain field. The surrounding developed lots are served by private wells.
There are no wells on surrounding lots within 100 feet of this system.
Soils. Two test holes were performed by Pannone Engineering Services in August of 2002, and
groundwater was monitored for seven days. Water was found at ten feet (10.0') in the test hole monitor
tube after the seven day monitoring period. Bedrock was not encountered in the test hole. It is my
opinion, based on the results of the percolation tests and overall soils appearance; an application rate of
0.6 gallons/day/square feet should be used for a conventional wastewater system.
1. Soil Absorption System Design.
a. See Sheet 2 of the design package.
2. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
3. Topography: The existing topography slopes from the southeast to the northwest at
approximately 10% in the area of the proposed drain field. There are no steep slopes in the vicinity of the
drain field.
4. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site

The proposed installation will not affect the future development of the surrounding or existing lots. There
are no surrounding wells within 100 feet of the proposed septic location.

Certificate of On -Site Systems Approval 4 141095
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
1 of McCready Meadows subdivision. This inspection revealed a nitrate
concentration of 5.47 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

SUBMIT STAMPED AND SIGNED RESULTS WITH THE AS -BUILT INSPECTION REPORT. IF THE RESULTS
REQUIRE A DESIGN CHANGE, CONSTRUCTION OF THE SYSTEM WILL STOP PENDING ONSITE REVIEW
AND APPROVAL.

I am writing to request a permit to construct new septic system be issued for this
property. The proposed systems will serve a proposed four-bedroom house. The lot is
approximately 62,160 square feet (1.472 acres) in size.
1. Soils. See the attached soils log. Ground water was monitored through the
seasonal high period of 2002. Ground water was measured in the monitor at a depth of
ten feet below ground surface in test hole 2. Bedrock was not encountered in the test
hole. It is my opinion that the overall soils appearance of the soils, an application rate of
0.6 gallons/day/square feet should be used using a standard treatment system.

3. Surface Water: There is no surface water within 100 feet of the proposed system.
4. Topography: The average topography in the area of the proposed septic system is
approximately 9 percent, based on topography developed in 2002. There are areas of 18
percent down hill of the system. There is an existing drainage ditch along Hillside Drive
to the west. The reserve field will be located approximately 38.6 feet uphill from the 2:1
slopes of the ditch. A section was developed for the most restrictive portion of the area
and it shows that the effluent will not daylight into the ditch.
S. Future Upgrade Area: I am proposing that the primary field and the reserve
fielded be installed at this time. I have shown the proposed location on the site plan.
The proposed installation will not affect the future development of the surrounding or
existing lots. There are no wells or septic systems within 100 feet of the proposed well
location or of the proposed septic location.
