All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
The Engineer is required to do an additional percolation test prior to construction of the septic field. Please
submit stamped and signed results with the As -built Inspection Report. If the results require a design change,
construction of the system will stop pending On -Site review and approval.

To Whom it may concern:
This is a request for a revised septic permit on the above referenced lot. Due to seasonal high water the old
bed system did not work therefore we are proposing a new design. A new test hole was excavated on the
uphill side and found the same material top to bottom, GM with water at 11 feet during excavation and after
the 7 day monitoring period. The lot has a slope change, with a maximum elevation change of 9 feet on the
east side and 6 feet on the west side, see the site plan. The new design will fill the slope area on the south
side with fill to a 2:1 slope to keep the effluent from day -lighting. A lift station has been added to the design
to allow the use of the good soils in the top of the test hole.
This new system will not impact any of the neighboring properties. The slope will be filled to less than 20
percent to the south, see the site plan.
