is 25 feet and to a community sewer line is 75 feet. Well logs are required

system in accordance with ADEC and MOA D~H~ Guidelines & Regulatiohs. The reported results described the
performance of the system under the conditions encountered at the time of the test, and separation distances
measured to readily identifiable features. The operational life of all wells and septic systems depend
on the local soils condition, ground water levels that may fluctuate during the year, and the water
usage of the family being served by the system. These conditions are outside the control of
the evaluator of the system. Satisfactoq/ test results do not guarantee future performance
of the system, nor do they guarantee that there are no hidden defects or encroachments.
AWWC, Inc. can therefore not provide any warranty for future estimate of how long the

Mr. Jeff Poet:
The ;veil head on the referenced property currently extends 6 to 10 inches above grade. The well
is located just off the drive;vay and is placed on a slope that is greater than 25 percent which has
excellent drainage. There are rocks placed around the well head which are currently frozen in
place. It is our opinion that the well casing is not a concern and that if needed, the rocks around
the well head may be removed in the future in order to have 12 inch minimum casing height
above grade.

Dear Mr. Garness:
This letter will serve to amend the subject waiver for the separation distances from the
on-lot .septic tank to water well and surface water. The current waiver allows the septic
tank to private water well of 72 feet and septic tank to surface water of 80 feet. The
amended waiver approves the septic tank to water well of 65 feet and the septic tank to
surface water of 75 feet.
This amended waiver separation distance approval applies to the existing septic tank to
water well and surface water only. Any future upgrade to the septic tank will require all
separations be met or another waiver approval from this office.

Dear Mr. and Ms. Crevistan,
Per your request we performed a well flow test and a septic adequacy test at the
referenced property on March 3, 2000, for purposes of obtaining a Health Authority
Approval (HAA). The results of our investigation are summarized as follows:
A. WELL: On the day of our test, 3/3/00 (the static water level in the well was
measured at 61.5 feet below the top of the casing (BTC). A meter was connected to the
outside hose bib and the flow turned on full. Based upon our readings and data over a
two hour period, it was concluded that the well production is approximately 5.8 gallons
per minute, with a fourteen fr. drawdown. The well flow production meets Municipality
of Anchorage (MOA) minimum flow requirements for a three bedroom dwelling, which is
.31 gallons per minute. It should be noted that seasonal fluctuations in the well
production can occur, and that future performance is not guaranteed.
B. SEPTIC: Per MOA records, the onsite wastewater disposal system consists of a 1000
gallon steel tank with a 196 gallon culvert type lift station followed by a 53 foot shallow
trench with an absorption area of 305 sq. fr.. On the day of our inspection (3/3/00) we
found the sump to be dry prior to the test. A total of 1500 gallons of water was added
directly in the sump causing a liquid level of 23 inches (one inch over the top of
distribution line). It was' also noted that the sump extends 17.5inches below the invert of
the drainpipe. (The inspection report notes an effective depth of 12 inches.) Eighty-three
minutes later, the liquid level was found to be at 13.5 inches, a drop of 9.5 inches. Based
on this data, we determined that the septic system meets the municipal absorption
requirements for a three bedroom house.
NOTE: The adequacy of a septic system is influenced by numerous factors, including,
but not limited to, seasonal surface water infiltration, groundwater variations, septic
system maintenance (frequency of septic tank pumping, usage of biological addilives)
conditions of drainpipe and pipe joints (which can be damaged by seismic activity and
deteriorate with age), type of substances deposited in septic system (cigarette butts,
sanitary napkins, misc. objects), and the amount of water being introduced on a
continual basis. Consequently, the results of this adequacy test are only valid for the
specific day of the test. Furthermore, because of the limited nature of this investigation,
it is possible that there are hidden defects which may not have been detected No
warrantee is made regarding the future performance of this well or septic system.
C. SEPARATION DISTANCES: Part of the HAA process is to verify separation
distances between the on-site well and septic. Our records research at the MOA found
there are two existing waivers on your property. A 72 ft waiver from your septic tank to
your well and a 80 fL waiver from your septic tank to the creek. After field verification
by our engineer teelmician, it appears the separation distance measurements are incorrect
and an amendment to the existing waivers will be necessary with the correct
measurements. The separation distance from the septic tank to the well is appoximately
65 feet. The separation distance from the septic tank to surface water is approximately 75
feet. If the MOA will not amend the waivers, we will need to apply for new ones.
Once you have a buyer for your property, we will need to take water samples and submit
all paperwork to the Municipality of Anchorage to obtain the Health Authority Approval
(HAA). The costs are as follows:

In regards to the at~ached letter from the State DEC and in accordance
with our previous telephone conversalion a high water alarm has been
i~talled in the lift station on the referenced property. Elevations
were checked between the discharge line from the house into the lift
station, the ground surrounding the lift station, and the crawl space
under the house. There is absolutely no doubt that in the event of
a failure of the lift station, effluent from the septic tank will back
up through the septic tank and into the crawl space, completaly flooding
the ~raw£ space before any possibility of the effluent overflowing the
lift station chamber. Your concern with the pumping cycles and dosing
rate were explored and it was found that by adjusting the low water/~gh
water ~ut offs on the pump that it is practical to increase the dosing
rate to approximately 45 gallons per discharge.
Request you approve the Health Authority Approval application as previously
requested.

The on-site wastewater disposal system and well on the referenced property
was installed in 1981. At that time the Municipality of Anchorage granted
a waiver to the horizontal separation distances between the on-site
well and the septic tank and lift station. They also granted a waiver
to the horizontal separation distances between the septic tank lift
station and a nearby creek. Attached for your review in addition to
the Health Authority Application and supporting data is a site plan
showing the general relationship between those utilities under waiver.
Special attention was given to the sealing of the septic tank and lift
station when they were installed. The topograpy in the area is such
that the vertical distances between the well and the septic tank are
relatively flat. The topography between the septic tank/lift station
and the creek is gently sloping towards the creek. Since the septic
tank and lift station are sealed units it is our opinion that they can
be treated as part of the conduit between the house and absorption area.
The soils between 25 feet and 70 feet consists of clay, gravel and boulders.
There is also a tight sand layer between 12 feet and 18 feet. Both
of these layers will protect the domestic water from contamination.
The static water level in the well casing is 60 feet below the top of
the well casing and well below the protective layers of soil.
It is our opinion that the horizontal separation distances prescribed
by 18 AAC 72.021 are not required in this case, therefore, request you
accept these waivers and issue the attached Health Authority Approval
certificate.
